EasyLiving is proud to be an approved provider participating in the GUIDE Program (Guiding an Improved Dementia Experience), helping families throughout Pinellas, Pasco, Hillsborough, and Polk Counties access additional dementia support services at no cost to eligible Medicare beneficiaries.
What Is the GUIDE Program?
The GUIDE Model was developed by the Centers for Medicare & Medicaid Services (CMS) to improve the quality of life for people living with dementia while reducing stress and burden on family caregivers.
The program focuses on comprehensive dementia care coordination, caregiver support, education, ongoing monitoring, and access to respite services. GUIDE is designed to strengthen dementia care in the community and help families navigate the aging journey with greater confidence.
Importantly, GUIDE does not replace a person’s existing doctors or healthcare providers. Instead, it adds an additional layer of support focused specifically on dementia care and caregiver needs.
What Benefits Does GUIDE Provide?
Eligible participants may receive:
Dedicated Care Navigation
Each family is connected with a dedicated Care Navigator who serves as a consistent point of contact. The navigator helps coordinate care, connect families with resources, monitor changing needs, and provide ongoing guidance.
Caregiver Education and Support
Family caregivers receive practical coaching, education, planning assistance, and resources designed to reduce stress and improve confidence when caring for a loved one with dementia.
24/7 Clinical Support
Questions and concerns do not always arise during normal business hours. GUIDE participants have access to clinical support around the clock, helping families address concerns before they become crises.
Respite Care Funding
One of the most valuable aspects of the GUIDE program is caregiver respite support.
Eligible families may receive up to $2,563 annually in respite care funding, helping caregivers take a break, attend appointments, travel, or simply recharge while ensuring their loved one remains safe and supported.
Ongoing Assessments and Monitoring
The GUIDE program includes structured assessments and regular touchpoints that help identify changes early and support proactive care planning. This allows concerns to be addressed before they become emergencies.

Who May Qualify?
In general, individuals may qualify if they:
- Have confirmed or suspected dementia
- Are enrolled in Traditional Medicare Parts A and B as their primary coverage
- Have an identified family or informal caregiver
- Are not enrolled in Medicare Advantage, PACE, hospice, or residing in a Skilled Nursing Facility (SNF)
Eligibility is determined through a screening and assessment process.

- Complete a comprehensive dementia care assessment
- Submit information for approval
- Begin receiving GUIDE services and support once enrolled
